Before filling out this form please be advised that unless you have a contact in the United States where funds can be sent by check, or you have a Vatican Bank account you will not be eligible to do Mission Appeals in the Archdiocese of Chicago. We no longer can transmit funds by wire transfer. MCP parish assignment information will be sent to your US Contact. He/She is responsible for notifying you.
